This is the registration information and form for the Otterbein Conference in July. The conference dates are July 11, 12 & 13, 2013.
These documents require Adobe Reader.
PRE-REGISTRATION IS REQUIRED.
A $25 late fee will be assessed for EACH APPLICATION RECEIVED after June 10, 2014 – NO EXCEPTIONS.
Rooms are based on double occupancy.
Rooms will NOT be assigned to walk-in participants. Rooms will be provided to registrants, if their registration is RECEIVED by June 30, 2014. People registering after June 30, 2014 will be given a list of area hotels at registration. Meals will NOT be provided to walk-in participants however, the registration fees will still be charged.
Send all forms and payments to the address listed on the form. Do NOT hold them to bring to me at the conference. Do NOT hold payments until arrival.
Rotarians, District leaders, O-E leaders, Country Contacts MUST register.
All adults and all students are requried by Otterbein University to submit a signed waiver.
